Meet our professional team:
Tom Skiba Administrator (not pictured)
Tom oversees the direction and coordination of all aspects of resident care, customer service, budgetary control and regulatory compliance.
Tom has over 23 years experience in skilled nursing becoming licensed as a Nursing Home Administrator in 1988. He joined the professional team at Twin Oaks Rehabilitation and Nursing Center in March 2012. Tom embraces a resident centered approach to caring.
Tom has a Bachelor of Arts Degree in Health Science from California State University, San Bernardino.

Hope Ruiz Activities Designee
Hope is in charge of a full service activity program. Hope has worked for Magnolia Health Corporation for over 17 years. She began her career as a Certified Nurse Assistant then gradually moved to different departments gaining beneficial experience. Hope received special training from the Clovis Adult School to enhance her abilities as the facility’s on-site Activities Designee.

Debbie Coleman, LVN (not pictured) MDS Designee
Debbie interviews family members, residents and caregivers to obtain necessary information for their care needs. She also ensures all federally mandated requirements are met.
Debbie has over 13 years experience in rehabilitation, short and long-term care. She also has vast expertise in hospice and home health care. Debbie joined the professional team at Twin Oaks in November 2010. Debbie received her degree as a Licensed Vocational Nurse from the Visalia Adult School.

Isabel Palomo, RN Director of Nursing
Isabel is in charge of overseeing all nursing responsibilities. She works closely with residents, family, physicians and nursing staff to assure all residents are safe and content as well as receive the best care and clinical outcomes possible.
Isabel has over 18 years experience in a skilled nursing setting. She graduated from College of the Sequoias as a Licensed Vocational Nurse and later graduated as a Registered Nurse from Pacific Union College in Angwin, California.

Trini Toft Social Services Designee
Trini works with residents and family members to ensure each resident’s transition into the facility is smooth and that their individuality is considered in all aspects of their care. Trini begins the process of planning for the resident’s future on the very first day of admission, so the entire team has a direction of care.
Trini has worked for Magnolia Health Corporation for over 11 years. She started as a Certified Nurse Assistant then became a Restorative Nurse Assistant. She has received valuable training as a Social Services Designee from the corporation’s other sister facilities.

Susan Alvarez Food Services Supervisor
Susan oversees quality of food, proper cleanliness and that every resident’s nutritional needs are met.
Susan has over 15 years experience as a Food Services Supervisor. She has worked in Magnolia’s sister facilities for over 10 years. Susan has received special training as a Dietetic Services Supervisor and Certified Professional Food Manager from College of the Sequoias.

Sandra Mathis, RN Assistant Director of Nursing
Sandra helps residents and monitors their conditions. She also helps with new admissions, physician orders, staffing and mandated federal regulations.
Sandra has over 15 years experience in skilled nursing. After working in a Magnolia sister facility, Sandra joined the professional team at Twin Oaks in June 2010. Sandra received her license as a Registered Nurse from San Joaquin Valley College.

Tia Alves, LVN Director of Staff Development
Tia provides staff training and in-service education. She also oversees and provides direction to Certified Nurse Assistants, monitors for infection control and ensures residents receive daily quality care.
Tia graduated from the Tulare Adult School as a Licensed Vocational Nurse in 2008. She first began her career in a Magnolia sister facility, and in 2010 joined the professional team at Twin Oaks.
